Metabolic Testing (VO2 Max)
Wayland Personal Physicians is proud to now offer Metabolic VO2 Max and Resting Metabolic Rate testing in our office for our Members!
Using the PNOE testing system, patients may elect to undergo resting as well as active metabolic testing, in conjunction with counseling from our team at WPP as well as the option for additional consulting with metabolic experts at PNOE.

Resting Metabolic Rate Testing
RMR (Resting Metabolic Rate) testing measures how many calories your body burns while you are doing absolutely nothing.
While a max test tells you how much “horsepower” your body has during a race, an RMR test tells you your body’s “idle speed”—the energy required just to keep your heart beating, your lungs breathing, and your brain functioning.
Why It Matters
For most people, RMR accounts for 60% to 75% of the total calories burned in a day.
- Precision Over Guesswork: Most people use online calculators to guess their calorie needs, but these can be off by 200 to 600 calories per day.
- Identify “Slow” or “Fast” Metabolism: The test proves whether your metabolism is actually running slower or faster than average for your age and weight.
- Fat vs. Carb Burning: It measures how much your body prefers to burn fat or carbohydrates for fuel at rest
How It Is Measured
The test is very simple and takes about 15 to 20 minutes.
- Preparation: You must fast (no food or caffeine) for at least 5 to 12 hours and avoid exercise before the test).
- The Test: You sit or lie down in a dark, quiet room. You wear a mask or a “canopy” hood that captures every breath you exhale.
- The Science: A machine called a metabolic cart analyzes the ratio of oxygen consumed to carbon dioxide produced. Because your body uses a specific amount of oxygen to burn one calorie, the machine can calculate your exact daily calorie burn.
Wellness and Weight Management
Knowing your RMR is like having a “metabolic GPS” for your health goals.
- Sustainable Weight Loss: It helps you set a calorie goal that is low enough to lose fat but high enough to keep your muscle and hormones healthy.
- Avoiding Plateaus: When you lose weight, your RMR naturally drops. Testing every 3–6 months allows you to adjust your diet so you don’t stop seeing progress.
Muscle is Engine Fuel: Since muscle tissue burns more calories than fat, you can use RMR tests to see if your strength training is actually speeding up your metabolism over time.
Active Metabolic (VO2 Max) Testing
VO2 max testing is considered the best way to measure how fit your heart and lungs are. It stands for the maximum amount of oxygen your body can use while you are exercising as hard as you can.
Think of it like a “horsepower” rating for your body. The higher the number, the better your heart, lungs, and muscles are at working together to power your movement.
Why is it important?
While pro athletes use this test to get faster, it is actually a great health tool for everyone.
- A “Vital Sign”: Doctors now look at it like your blood pressure or heart rate because it tells them how healthy your internal systems are.
- Easier Movement: When your VO2 max is higher, daily activities like playing sports, hiking, or even carrying a heavy backpack feel much easier.
- Predicting Health: People with higher scores generally have a lower risk of getting heart disease or other long-term health problems.
How is it measured?
To get a perfect measurement, you have to go to a lab or a specialized gym.
- The Gear: You wear a special mask that covers your mouth and nose. This mask is connected to a machine that measures every breath you take.
- The Test: You run on a treadmill or ride a stationary bike. Every few minutes, the workout gets harder and harder.
- The Limit: You keep going until you are breathing as hard as possible and can’t go any further. The machine calculates how much oxygen you used at your absolute peak.
Longevity and Wellness
VO2 max is one of the best ways to predict “longevity,” which is a fancy word for living a long and healthy life.
- Staying Young: As people get older, their fitness naturally starts to drop. However, by keeping your VO2 max high through exercise, you can keep your body “younger” and more capable even as you age.
- Brain Power: Some studies show that being fit helps your brain stay sharp and healthy.
Better Energy: A high VO2 max means your body is great at turning food and oxygen into energy, which keeps you from feeling tired all the time.
