Why Monitor Your Blood Pressure from Home?
If you have high blood pressure, you know the importance of monitoring it. As a member of Wayland Personal Physicians, you can use our home blood pressure monitoring program to help us provide better care for you.
High blood pressure, known as hypertension, is one of the leading risk factors for cardiovascular disease in this country. Home blood pressure monitoring allows you to obtain more blood pressure readings during the year, in the 'real world', enabling us to better assess your blood pressure control.
Through home blood pressure monitoring, together we can improve communication and coordination of your home readings and proactively make changes to your blood pressure medications without always needing to come into the office. We have partnered with Welch Allyn, a well-known and respected blood pressure device manufacturer who provides the machines used in our office, to check your blood pressure.
If you choose to participate using the Welch Allyn 1500 or the Welch Allyn 1700 (recommended) home blood pressure monitors, you can upload your blood pressure readings to your Bluetooth-enabled smartphone automatically and share them with us through a secure, web-based portal. This allows us to monitor your blood pressure readings remotely, without you needing to keep a written list or taking the time to email us your readings. You will also have access to the free Welch Allyn smart phone app, which can sync data with either the Apple health kit or the Android health app.
Your personal physician will monitor and review your uploaded blood pressure readings to assess your level of blood pressure control.
How to Get Started with Home Blood Pressure Monitoring
As a member of Wayland Personal Physicians, you have access to our home blood pressure monitoring program at no additional cost. You may purchase a blood pressure machine through our office at cost, or you are welcome to purchase the cuff on your own if you prefer. We do not receive any finanical incentives from Welch Allyn.
